Periodic alternating nystagmus (PAN) is characterized by a periodical reversal in the direction of the nystagmus. Acquired PAN has been reported in association with a number of conditions, many of which involve the crerebellum. A 40-years old woman presented with periodic vertigo and PAN but had no other neurological signs. Anti-GD1b IgG and anti-GM1 IgG antibody were detected. It is rare case that isolated PAN associated with antiganglioside antibodies. Antiganglioside antibodies syndromes should be considered in patients with nystagmus of undetermined etiology.
